We're here to help individuals in their journey towards recovery with medical treatment and wrap around support. 

WHO SHOULD APPLY?

  • Individuals currently struggling with opioid addiction.

  • First-time Sublocade injection users that wish to stop opioid use altogether.

  • Those willing to participate in counseling and follow-up support during the program period.

All information submitted through this form will be kept confidential and used solely for the purpose of evaluating eligibility.

PROGRAM OUTCOMES:

Why Sublocade?

Research shows that Sublocade is an effective, evidence based choice for the use of opioid use disorder.

The FDA has approved Sublocade for treating moderate to severe opioid use disorder in adults

Patients treated with long acting injectable buprenorphine had, on average, about a 40% abstinence rate from illicit opioids (such as heroine or fentanyl), compared to about 5% among patients on placebo.

WHAT TO EXPECT AFTER APPLYING

1. Submit your application​​

Complete the form with honest and accurate information about your situation and treatment goals.

2. Application review

Our team will review applications and may contact you for additional information.

3. Selection and outreach

If selected for the pilot program, you will be contacted directly with next steps for scheduling, treatment coordination, and support services.

4. Begin support services

Participants approved for the program will receive assistance coordinating Sublocade treatment along with recovery and financial counseling during the support period.

SUBOXONE TO SUBLOCADE

If you are currently on Suboxone and considering switching to Sublocade, learn more about the process, what to expect, and answers to common questions.

ELIGIBILITY GUIDELINES

  • You are 18 years of age or older

  • You are able to participate in the follow up counseling and support services

  • You are seeking treatment for opioid use disorder

  • You are willing to work collaboratively with healthcare providers and the Wesley's Reach team

ABOUT THIS APPLICATION

This application is for individuals seeking support through Wesley's Reach's Sublocade Pilot Program. Our program is designed to help individuals strugglign with opioid addiction begin treatment with Sublocade, a once a month injectable medication. 

Selected Participants Will Recieve:

$500 for the first injection, covering a minimum 30-day commitment

Recovery counseling and support for 30 days

Potential eligibility for an additional 30 days of continued support

Note: Completing the application does not grantee selection into the pilot program. Applications are reviewed carefully to identify candidates that are ready to engage in treatment and who may benefit most from the support Wesley's Reach provides.

While honesty is paramount in our methods and process; as a former addict myself. I understand there are times where a vague answer is the best answer one can provide given the nature or topic of a certain question.


 Please just be as honest as possible within reason and keep in mind that every single person within our non-profit has been through the opioid addiction recovery process.
